Handling Chronic Diseases

In my area of nursing practice, the management of chronic diseases is a multifaceted approach that involves patient education, medication administration, lifestyle counseling, and regular monitoring. We follow practice guidelines established by both federal and provincial health authorities. These guidelines are consistent with best practices and help ensure that patients receive evidence-based care. For example, in managing diabetes, we follow the Canadian Diabetes Association's clinical practice guidelines to provide standardized and effective care (Sockalingam et al., 2020). Additionally, for hypertension management, we adhere to the recommendations provided by Hypertension Canada. These guidelines guide our assessments, interventions, and monitoring protocols, ensuring that our practices align with the latest evidence and best practices in the field. By following these guidelines and staying up-to-date with advancements in healthcare, we aim to provide the highest quality care to our patients while effectively managing chronic diseases.
